Identity And Work Eligibility verified.
State Criminal Background Check completed under HRS section 321-15.2.
Tuberculosis Clearance under HAR Chapter 11-164.
Personal Care Aide Qualifications, with our guidance and training support.

Please do not email sensitive identity documents. Applicants still visit the office for an ID photo and contract signing.
